I consider Bionic Commando pretty fucked up, only cause I was around 10 when I played it and got to see Hitler's face explode. The same goes for the SNES Battletoads, where you get sawed in half if you lose in the rollercoaster level.

Yume Nikki is the first one to come to mind, but I Have no Mouth and I Must Scream was probably worse. Silent Hill 2 is a good option too. But Harvester has wasps coming out of a baby's eyes, a guy with a meat fetish and more, plus, the fmv gives it that estra b movie feeling, so it definitly wins.

I haven't played it either, but Xenophobia's demo is promising, it's the game the guy from Demonophobia is doing now. It has some sounds, and a few details, like light and bleeding.
